Providing patient care in their own homes and in hospitals.
To work within a delegated caseload with the focus of facilitating behavioral change to promote and optimize self\-care.
To communicate information to patients and carers regarding their health needs and associated treatment plans.
To report and take appropriate action regarding any change in patient condition or circumstance and communicate verbal and written information to all relevant integrated care team members.
To contribute to and support the maintaining of service standards and performance through audit and bench\-marking, and to encourage colleagues and team members to do also.
To participate, as an individual and as team member, in continuous learning and reflective practice to maintain own knowledge and skills, share good practice with and provide instruction to colleagues to continually improve services.
To continue to promote patient centered care and utilize the shared decision making tools.
To actively contribute to protecting people at risk by assessing and monitoring the care environment and individuals and by following and acting on and within Safeguarding policies and procedures.
We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients’ homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.
We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children’s and young people’s services; end of life care and outpatient and diagnostic imaging services.
We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

The overall aim of the service is to work with individuals to increase their knowledge, skills and confidence to manage their long\-term conditions and care needs, This role initially will pay particular emphasis to diabetes, however all long term conditions will be supported.
This is a clinical role and will involve care delivery with a behavioral change focus.
You will work closely with individuals to empower independence with their health and social care needs and use the Goal setting approach to help achieve this.

To work as part of a multidisciplinary team supporting the care of patients within the community.
To undertake the administration of medications that are prescribed for patients on a regular basis.
To promote self\-care and education to patients.
Be able to create a professional culture which is open to change, new ideas, concepts and innovation whilst reflecting organisational strategy and values.
Clinical duties
The post holder will work within an agreed framework to plan, implement, and evaluate clinical and/or therapeutic activities and patient responses to activity.
Support and assist other staff members to deliver care as delegated and directed.
Demonstrate highly developed physical skills obtained through practice and training e.g., accuracy, dexterity and coordination to complete tests and assessments within agreed pathways of care and manipulate equipment.
The post holder will be able to make judgement involving facts or situation which may require analysis e.g., alter programmes of activities or making clinical decisions related to specific pathway of care.
Able to identify risk or changes in condition and is responsible for acting; advises supervising professional and accurately record all interventions.
Provides and receives routine or sometimes complex information using persuasion, reassurance, tact and empathy and addressing barriers to understanding in order to engage the individual in their care and gain valid consent for care, record discussions in an accurate and timely manner in the individual’s patient record.
Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of their role.
Engages and actively involves the individual and, if appropriate, their family/carer, in their assessment, programme of treatment and/or care by using person\-centred techniques to promote a culture of self\-care, involvement and empowerment.
Prioritises their own workload and understands the boundaries of their practice; may plan tasks for others e.g., patients, the workload of more junior staff.
Be responsible and accountable for clinical administration tasks and management of resources within the clinical area e.g., stock control, basic equipment cleaning, specific clinical tasks to support the smooth running of a clinic.
To comply with the organisations’ Infection Prevention and Control requirements, including “bare below the elbows” dress code for staff with a direct care delivery role.

Professional Duties
Have demonstrated the proficiency to administer medications and undertake annual medicines management training.
To be aware of and adhere to Trust Policies relating to the control and administration of drugs.
To ensure that high standards of care are given and maintained and to act when standards are not being maintained.
To work within the policies, procedures and guidelines of Calderdale \& Huddersfield Foundation Trust.
To participate in regular staff meetings.
To be flexible to work within areas or departments to meet the demands of changing service needs.
To provide mentorship and preceptorship to newly qualified staff and students, new starters and Trainee Nursing Associates encouraging an effective learning environment.
To provide support and supervision to Health care support workers to promote their learning and development within role.
To receive delegated duties from the supervising RN and senior colleagues, accepting those that fall within their individual competency level adhering to the NMC Delegation and Accountability tool (NMC Code 2018\).
To delegate appropriately to other team members, including Health Care assistants, family members and other lay carers adhering to the NMC Delegation and Accountability tool (NMC Code 2018\).
Recognize and work within the limits of their competence.
To take responsibility for own continuing professional development and performance.
Ensure that a professional service and image is maintained at all times, thereby acting as a role model to all staff.
Ensures the effective and efficient use of resources within their own sphere of responsibility.
Takes responsibility for their own and others’ health and safety in the working environment.
Assist the audit department with compiling audit questionnaires by using knowledge of areas to be audited. Use audit tools provided by audit department to collect data.
Participate in group reviews of services and areas as requested.
To monitor that all policies relating to medicines are followed.
Work with the team to produce meaningful reports on yearly /ad hoc basis.
To participate in regular reviews of medication errors and assist and decide action plans within the reviews.
