In partnership with the multidisciplinary team the post holder will be part of the provision of a TB Specialist Nursing Service, delivering high quality clinical care.

The post holder will develop specialist knowledge and skills and be a key clinician in the provision of advice, education and support to colleagues, patients, families and carers. They will participate in clinical audit and service evaluation within the specialist field. They will also be responsible for contributing to the on\-going development of clinical and community practice, standards of care within the service including development of policies, procedures, protocols and guidelines.

The post holder will participate in the delivery of educational programmes for patients and Multidisciplinary team. This post involves combination of nurse\-led screening clinics and community (visiting patients within their homes).

This role involves carrying out home visits as part of the service. Therefore, applicants must possess a valid UK driving licence and have access to a reliable vehicle to fulfil the requirements of the post.

Based in Derby, this role provides specialist case management for patients undergoing tuberculosis (TB) treatment, their families, and individuals attending TB screening services. Key responsibilities include assessing patients for drug resistance, monitoring treatment adherence, coordinating Directly Observed Therapy (DOT), and planning care in line with national and local guidance. The post holder undertakes contact tracing, TB screening, BCG vaccination clinics, and monitors treatment side effects, seeking specialist advice when required.

Working primarily within the community, the role involves delivering education and support to patients, carers, and healthcare professionals, while collaborating with multidisciplinary teams. Responsibilities also include requesting and reviewing diagnostic tests, contributing to outbreak management, attending MDT meetings and cohort reviews, managing a caseload, and ensuring accurate reporting to the National TB Surveillance Database.

The post holder carries out patient assessments, develops personalised care plans, supports discharge planning, promotes informed decision\-making, safeguards vulnerable individuals, advocates for patients, and maintains high standards of communication, handover, and clinical documentation in accordance with Trust policies and professional standards.
