Support Worker
Job Summary:
Your role is to support people with the things that matter to them most. The focus of this role is to empower individuals to live as independently as possible, ensuring their safety, facilitating decision\-making, and enabling them to lead fulfilling lives within their community.
You will provide support within a person’s own home and the nature of the support you provide will vary depending on each person's unique needs, with an emphasis on collaborative efforts rather than doing things on their behalf.
Your role is to promote and facilitate practices thatpromote individuals’ independence.
In addition to day\-to\-day support, you will also assist with household tasks such as cooking and cleaning, accompany them on outings, facilitate their participation in activities, and support their educational or work\-related endeavours, as well as their holiday plans.
As a member of a compassionate and close\-knit team, operating within a specific service, you will receive dedicated management support and we will equip you with the necessary skills and provide ongoing mentoring to ensure that you can perform your job to the best of your abilities.
Duties:
Daily living and independent skills
Helping people to get organised, plan their day, attend employment, household cleaning tasks, meal\-plan and shop.
Socialising
Helping people meet friends and stay in touch with loved ones.
Supporting communication
Assisting individuals who may face challenges in communication by ensuring information is easily understandable, empowering them to communicate their thoughts and preferences.
Keeping active
Accessing the community and taking part in activities such as swimming, gym classes, music concerts,dance/drama classes, cinema, theatre trips, football.
Medicine management
Working with health professionals to support any medical needs, including making and attending any necessary appointments or administering medication.
Budgeting
Including day to day money management and financial support.
Goals and aspirations
Supporting people to achieve their short and long terms goals, which could be anything from planning and cooking a meal, to attending college, finding paid employment, or going on day trips and holidays.
Delivering individual support plans
Following a dedicated person\-centred support plan. Always involving the person you support
Providing emotional and behavioural support
Being on hand to guide and advise, listen, and understand, build confidence and self\-esteem. Keeping the people we support safe at home and in the community.
