Contract Type: Full\-Time / Part\-Time, Permanent and Bank
Salary Range (South West Benchmark 2026\): £24,454 – £25,974 per annum (DOE)
Hourly Rate Equivalent: £12\.71 – £13\.50 per hour
Overtime Rate – Additional £1\.50 per hour.
Reporting to: Senior Support Worker / Deputy Manager / Registered manager

Role Purpose
The Support Worker delivers high\-quality, person\-centred care and support within a supported living service accommodating to service users living in their own homes. The role promotes independence, dignity, safety and community inclusion in line with CQC Fundamental Standards and Blossoming homes’ organizational values.
The post holder supports individuals with autism, learning disabilities, and additional health conditions or associated needs, enabling them to achieve personal goals, maintain wellbeing, and live fulfilling lives at home and in their community.

Core Duties \& Responsibilities
1\. Person\-Centred Care Delivery
\- Provide practical and emotional support tailored to individual support plans.
\- Assist with personal care, which may include providing assistance with tasks such as bathing, dressing, toileting, and eating, while always promoting dignity, independence, and choice.
\- Assist with daily living tasks and household responsibilities as required.
\- Support service users to develop life skills and independence such as budgeting, shopping, managing benefits, and household tasks.
\- Encourage participation in social, educational and community activities such as swimming, arts and crafts, walking, or other hobbies the person enjoys
\- Promote dignity, respect and choice in all interactions.
\- Accompanying individuals to appointments such as GP, dentist, opticians, hairdressers, or other professional visits, ensuring they feel supported, understood, and confident throughout.
\- Supporting individuals to access holidays, day trips, and breaks away from home when needed, helping with planning, preparation, and accompanying them to ensure a safe and enjoyable experience.
2\. Safeguarding \& Safety
\- Maintain vigilance in identifying safeguarding concerns.
\- Follow safeguarding procedures and report concerns immediately.
\- Support safe medication administration in accordance with policy.
\- Follow risk assessments and safe systems of work.
\- Contribute to maintaining a safe, clean and homely environment.
3\. Documentation \& Compliance
\- Complete accurate daily notes and documentation in line with policy.
\- Record incidents and report concerns promptly.
\- Participate in audits and quality monitoring processes.
\- Maintain confidentiality and comply with data protection requirements.
\- Support compliance with CQC standards and organisational policies.
4\. Positive Behaviour Support
\- Apply Positive Behaviour Support (PBS) principles in daily practice.
\- Support proactive strategies to reduce incidents and restrictive practices.
\- Respond calmly and professionally to behaviours of concern.
\- Contribute to learning reviews following incidents.
5\. Teamwork \& Professional Development
\- Work collaboratively with colleagues and leadership team.
\- Attend team meetings and training sessions.
\- Engage in supervision and appraisal processes.
\- Contribute to a positive, inclusive and supportive team culture.
\- Maintain professional boundaries at all times.

Qualifications
Essential:
\- Willingness to complete Care Certificate within probation period.
\- Basic literacy and numeracy skills.
Desirable:
\- Level 2 or Level 3 Diploma in Health \& Social Care (Adult Care).
\- Medication administration experience
Experience
Essential:
\- Experience working in a care or support environment (paid or voluntary).
\- Experience supporting individuals with additional needs.
Desirable:
\- Experience supporting individuals with Learning Disabilities or autism.
\- Experience working within supported living environment.
\- Experience supporting individuals with complex needs or behaviours of concern.
