Palliative Care Assistant
Location: STOKE/ STAFFORDSHIRE Community\-based / Clients' Homes
Job Description:
We are looking for Care Assistants to provide compassionate, person\-centred care and support to individuals with life\-limiting illnesses who wish to remain in their own homes.
The role involves assisting clients with personal care, promoting comfort and dignity and supporting emotional well being. You will work closely with families and healthcare professionals to deliver high\-quality palliative and end\-of\-life care.
Due to the nature and sensitivity of this role, we are looking for Care Staff with an empathetic and gentle approach to the care they deliver.
Key Responsibilities
· Personal Care
· Assist clients with washing, bathing, dressing, grooming, and toileting.
· Support with continence care and maintaining personal hygiene.
· Assist with mobility, positioning, and transferring using appropriate equipment and techniques.
· Monitor and report changes in clients' condition, comfort, or well being.
· Support clients to remain comfortable through repositioning and other non\-clinical comfort measures.
· Assist with nutrition, hydration, and meal preparation according to care plans.
· Support medication administration.
· Provide companionship and emotional reassurance to clients and their families.
· Respect clients' wishes, beliefs, culture, and dignity at all times.
· Offer sensitive support during end\-of\-life stages.
· Communicate effectively and compassionately with family members and carers.
· Provide practical support and guidance within the scope of the role.
· Recognise when additional professional support may be required and report concerns appropriately.
· Maintain accurate and timely care records.
· Report concerns, incidents, safeguarding issues, and changes in health promptly.
· Follow organisational policies, procedures, and regulatory requirements.
· Work collaboratively with nurses, GPs, hospice teams, district nurses, and other healthcare professionals.
· Attend team meetings, supervision sessions, and training as required.
· Contribute to the continuous improvement of care services.
Previous experience in domiciliary care, health care, or social care is required for this role.
A full UK License with Business Insurance on your vehicle is also desirable.
