Outreach Worker- telford

We are looking for a caring, reliable, and enthusiastic Outreach Worker to support a young lady to access and enjoy activities within her local community.

This is a rewarding role where you will help promote independence, confidence, and wellbeing by providing one\-to\-one support tailored to her interests and goals.

Key Responsibilities

• Support the young lady to access community activities, appointments, and social opportunities.


• Encourage independence while providing appropriate support.


• Assist with planning enjoyable and meaningful outings.


• Provide safe, person\-centred support at all times.


• Maintain accurate records and communicate effectively with family and the wider support team.


• Follow care plans and risk assessments.

Essential Requirements

• Full UK driving licence and access to a reliable vehicle (essential).


• Good knowledge and understanding of epilepsy, including recognising seizures and responding appropriately.


• A caring, patient, and positive attitude.


• Excellent communication and interpersonal skills.


• Reliable, flexible, and able to work independently.


• Previous experience supporting people with additional needs or disabilities is desirable.
