Outreach, Navigation and Support Worker
To provide an integrated outreach, navigation and support service to people experiencing homelessness or at risk of homelessness, working proactively with individuals in the community, on the streets and within accommodation to identify needs, reduce barriers and support people towards safe, sustainable accommodation and improved wellbeing.
The post holder will build trusting relationships with people who may have complex needs, providing practical and emotional support, advocacy and navigation into appropriate services. They will work closely with housing, health, substance misuse, mental health, statutory and voluntary sector partners to coordinate effective interventions and achieve positive outcomes.
Duties and Responsibilities
1\. Outreach
- Undertake proactive outreach to people sleeping rough or at risk of homelessness.
- Build relationships with people who may be reluctant or unable to engage with traditional services.
- Complete initial assessments and identify immediate risks and support needs.
- Respond appropriately to safeguarding, welfare and risk concerns.
- Provide practical support including access to food, clothing, identification, benefits and healthcare.
- Work flexibly across early mornings, evenings and weekends where required.
- Coordinate and recruit staff and volunteers for the rough sleeper outreach.
- Maintain accurate records of outreach activity and outcomes.
- Act as a consistent point of contact for allocated individuals.
- Develop individualised action/support plans with each person.
- Navigate individuals through housing options and statutory services.
- Liaise with Housing Options, local authorities, health services and voluntary\-sector organisations.
- Advocate for individuals where appropriate.
- Identify and remove barriers preventing people from accessing accommodation and support.
- Develop relationships with accommodation providers to create appropriate move\-on opportunities.
- Monitor progress and review support plans regularly.
- Provide person\-centred support to individuals within accommodation and in the community.
- Support people to develop the skills required to maintain accommodation.
- Assist with benefits, budgeting, appointments, correspondence and access to services.
- Support individuals to engage with health, mental health and substance misuse services.
- Encourage independence, resilience and positive decision\-making.
- Support individuals to develop routines, social networks and meaningful activity.
- Work alongside colleagues to manage risks and respond to changing needs.
- Develop effective relationships with statutory and voluntary agencies.
- Attend case conferences, multi\-agency meetings and partnership meetings.
- Coordinate interventions to prevent duplication and ensure individuals receive appropriate support.
- Share relevant information appropriately and in accordance with confidentiality and information\-sharing procedures.
- Develop knowledge of local services and maintain an up\-to\-date referral pathway.
- Maintain accurate and timely case records.
- Monitor individual progress against agreed outcomes.
- Collect information required for contractual, monitoring and funding purposes.
- Produce case studies and outcome information where appropriate.
- Contribute to service reviews and continuous improvement.
- Ensure all work complies with relevant legislation, policies and contractual requirements.
- Safeguarding is the responsibility of everyone within the organisation, and the post holder is expected to remain vigilant and act in the best interests of service users at all times.
- To complete all mandatory safeguarding training and maintain an appropriate level of knowledge and understanding in relation to safeguarding responsibilities.
- To recognise, respond to and report safeguarding concerns promptly and appropriately, in accordance with organisational policies, procedures and statutory requirements.
- To work positively and effectively across diverse communities, ensuring that all individuals are treated fairly and with dignity and respect.
- To demonstrate an understanding of and respect for individual differences, including people's backgrounds, cultures, beliefs, identities and circumstances, and to promote equality of opportunity in all aspects of the role.
- To undertake relevant equality, diversity and inclusion training and apply learning appropriately within day\-to\-day practice.
- To comply with all organisational Health \& Safety policies, procedures and safe working practices, taking reasonable care of their own health and safety and that of others who may be affected by their work.
- To undertake all required Health \& Safety training and ensure that learning is applied appropriately within their day\-to\-day responsibilities.
- To promptly report and accurately record accidents, incidents, hazards and other Health \& Safety concernsin accordance with organisational policies and procedures.
- To participate in appropriate training as and when required, including training relating to individual employee responsibilities for Health \& Safety.
- To handle all personal and confidential information responsibly and securely, ensuring that data is accessed, used, stored and shared appropriately and in accordance with GDPR, data protection legislation and organisational policies.
- To ensure that all relevant information and records are accurately, appropriately and securely recorded, maintaining confidentiality and ensuring that information is only shared with authorised individuals or agencies where there is a legitimate reason to do so.
