This is an exciting opportunity to play a key role in the delivery of an innovative 12\-month pilot programme supporting individuals with learning disabilities in Billericay to access Annual Health Checks and to take meaningful action following these through personalised Health Action Plans.

Your impact: You will support individuals to engage with their health and help them achieve practical improvements in their wellbeing, working hands\-on in this community\-based role.

About you:

  • You have experience working with people with learning disabilities

  • You have experience supporting individuals in a care, health or community setting

  • You have an understanding of person\-centred approaches

  • You have strong communication skills

  • You are able to build trusting relationships with individuals and families

  • You are able to work independently and as part of a team

  • You are compassionate, patient and have a supportive approach
