Integrated Discharge Team Nurse
The Discharge Nurse works within the Integrated Discharge Team working across the County Hospital (acute wards) and Community Hospitals (Bromyard, Leominster and Ross) in a collaborative way with Social Care colleagues to ensure the safe and timely discharges of patients that require support on discharge. Ensure that the team ethos is adhered to by all who are involved in the discharge process at all times, ‘Why not Home, why not today’. This will involve holding a caseload of patients with Nursing needs that require support on discharge.
