Heart Failure Specialist Nurse (Virtual Ward)
An exciting band 6 opportunity for registered nurses who have experience in cardiology has arisen in our heart failure virtual ward which is part of Dudley Heart Failure Team. The successful applicant will be responsible for the management of patients with heart failure over a 7 day service. This may also include in\-reaching into hospital in order to provide a seamless service.
This will involve direct face to face and virtual monitoring to provide acute patient management with a confirmed heart failure diagnosis in the community setting rather than hospital. The virtual ward will be a safe and efficient alternative to NHS bedded care that is enabled by technology. Patients will be admitted onto the virtual ward on a case by case basis for up to a 14 day period.
The service aim is to facilitate a reduction in patient length of stay; a reduction of hospital admissions/readmissions and assist early discharge from hospital by expediting safe and timely pro\-active support.
The post holder will be responsible for the delivery of comprehensive clinical assessment and care aimed at improving quality of life, supporting and empowering patients and their carers to manage their condition
The post holder will work as a specialist practitioner within the Dudley Heart Failure Team Virtual Ward, assisting in the provision of a comprehensive service to an allocated caseload of patients with Chronic Heart Failure. This may also include in\-reaching into hospital and out\-reaching in the Community in order to provide a seamless service.

The post holder will be responsible for the delivery of comprehensive clinical assessment and care of patients admitted on the Virtual Ward. This is aimed at improving quality of life, supporting and empowering patients and their carers to manage their condition .
