Hours: 2 x 35hrs or 3 x 17\.5hrs per week, temporary for 12 months

Salary / Hourly Rate: full time position: £38,779 \- £39,910, part time position: £19,389 \- £19,995

An opportunity has arisen for a highly motivated individual to join our Hospital Discharge Social Work Team.

You will be working with Adults aged 16 years and older who will have a range of health and social needs and play a vital role in our Frailty Assessment Units in University Hospital Wishaw and University Hospital Hairmyers working on a 2 week rolling rota.

These posts are temporary for 12 months.

You will be responsible for the completion of a holistic assessment of risks and needs, developing care support plans to meet identified needs, reviewing existing plans and supporting duty work under the supervision of the Operations Manager.

This work is underpinned by the principles of the Social Work (Scotland) Act 1968\. The successful candidate will be motivated, organised, willing to learn and have experience of working with vulnerable groups in the community.
