Clinical Pharmacist (Full time )
We have an exciting opening for a Clinical Pharmacist to join our well\-established general practice who has past experience in working in a primary care setting.
About the Role
You will play an integral part in the clinical team, working alongside GPs and other healthcare professionals to improve medication\-related outcomes for patients.
Key Responsibilities
Medication Reviews and Management
- Conduct structured medication reviews with patients.
- Review and manage repeat prescriptions efficiently.
- Carry out medicine reconciliation following hospital discharge.
- Monitor and respond to MHRA and CAS alerts appropriately.
- Undertake medicines management audits.
- Contribute to prescribing quality and cost\-efficiency initiatives.
- Support chronic disease management through appropriate medication advice.
- Act as a specialist resource for medicine\-related queries from colleagues and patients.
- Liaise with community pharmacies, secondary care, and other healthcare professionals.
- Signpost patients to other services where appropriate.
Essential Requirements:
- Degree in Pharmacy (MPharm or equivalent).
- GPhC registration as a Pharmacist.
- Experience working in General Practice (e.g. PCN pharmacist)
- Can commute to medical centre
- Familiarity with SystemOne consultation software.
- Postgraduate clinical training or an independent prescribing qualification.
