Job overview
The Hepatology Surveillance Nurse, Band 6, will collaborate closely with the nurse manager and the Band 8 nursing team to establish and implement a nurse\-led liver surveillance pathway. This pathway will support patients with stable, compensated liver cirrhosis who are at risk of developing Hepatocellular carcinoma (HCC) across various outpatient settings in Epsom and St Helier NHS Trust.
The primary purpose of the role is to deliver the HCC surveillance pathway.
Key responsibilities include: conducting patient assessments, ordering investigations such as blood test and Ultrasound scans (USS), reviewing blood and ultrasound results, and providing lifestyle advice and signposting and performing fibroscan. The aim of the pathway is early detection of HCC and effective management of liver disease to enhance patient outcomes and provide potentially lifesaving interventions.
The Hepatology Surveillance Nurse will work closely with the Gastroenterology/Hepatology nursing team to coordinate care for patients referred to the HCC surveillance pathways. This includes conducting nurse\-led clinics for individuals with stable, compensated cirrhosis and referring patients with advanced liver disease to senior Band 7/8 nurses and consultant clinics and nurse\-led fibroscan clinic. The nurse will be responsible for requesting and coordinating six\-monthly diagnostics for patients with compensated cirrhosis and tracking and monitoring patients throughout the pathway.
Main duties of the job
1\. To provide nurse led HCC surveillance clinics for patients with stable cirrhosis. Provide advice, information and support for participants enrolled in the liver HCC surveillance pathways. The responsible for care delivery for patients undergoing liver surveillance for stable compensated cirrhosis and to meet the needs of patients.Be competent to autonomously support patients to make informed decisions prior to undergoing a surveillance USS and communicate test results and implement surveillance and follow\-up. Manage patients within the liver cancer surveillance pathway in accordance with relevant society and National guidance (e.g., NICE QS152\).To use specialist knowledge and advanced communication skills with patients and carers when obtaining consent for procedures and providing psychological support where needed.Acting as the patient advocate supporting them to navigate and make informed choices throughout the process. Ensure timely access to USS surveillance for those individuals diagnosed with advanced fibrosis or viral hepatitis who are at risk of liver cancer. To establish effective links with Gastroenterology/Hepatology multi\-disciplinary team (MDT) in order to ensure that when cancer is detected, there is timely presentation and transfer of patients to the MDT. populate and maintain the Liver surveillance spreadsheet/database. Accurate use of Microsoft office, NHS net, local databases, remote connectivity in order to practice effectively and independently.
