Care Co-ordinator
Red Apple Care Ltd is seeking an organised, compassionate and motivated Part\-Time Care Coordinator / Care Support Worker to support the effective coordination and delivery of high\-quality domiciliary and supported living services. This role combines office\-based coordination duties with hands\-on care support responsibilities to ensure our clients receive safe, person\-centred care.
The successful candidate will play a key role in staff scheduling, communication, client support and care delivery while also participating in a fortnightly rotating on\-call rota.
We are looking for someone who is reliable, proactive, flexible and highly confident using computer systems and digital technology.
Main Duties and Responsibilities Care Coordination Duties
- Coordinate and schedule care staff rotas to ensure effective coverage of care visits and support shifts.
- Monitor and manage daily staffing requirements and respond to changes in care needs.
- Liaise with care staff, clients, families and healthcare professionals.
- Support with staff allocation and shift adjustments where required.
- Assist with onboarding new clients and setting up care packages.
- Maintain accurate client records and update care management systems.
- Monitor compliance documentation and ensure records remain current.
- Support with incident reporting and follow\-up actions.
- Assist with quality assurance and service improvement activities.
- Provide general administrative support as required.
- Deliver high\-quality person\-centred care and support to clients in their homes and supported living settings.
- Assist individuals with:
- Personal care
- Medication support (where trained and authorised)
- Meal preparation
- Mobility support
- Social and emotional support
- Daily living activities
- Promote dignity, independence, choice and wellbeing.
- Observe and report any concerns regarding clients' health or welfare.
- Participate in an on\-call rota which operates on a rotating basis every fortnight.
- Respond appropriately to emergency situations, staffing issues and client concerns outside normal working hours.
- Arrange cover for unplanned staff absence where necessary.
- Escalate concerns to management when required.
- Remain professional and responsive while on\-call.
- Previous experience in health and social care, preferably within domiciliary care and/or supported living.
- Experience coordinating rotas or managing schedules would be advantageous.
- Strong computer and IT skills; confident using:
- Email systems
- Microsoft Office (Word, Excel, Outlook)
- Care management software
- Mobile apps and digital systems
- Excellent communication and organisational skills.
- Ability to prioritise workload and work under pressure.
- Flexible and willing to undertake both office\-based and care duties.
- Ability to work independently and as part of a team.
- Full UK driving licence and access to a vehicle (if required).
- NVQ/QCF Level 2 or Level 3 in Health and Social Care.
- Knowledge of CQC regulations and standards.
- Experience within domiciliary care scheduling systems.

Pay: £12\.71\-£13\.00 per hour
