Band 5 Community Staff Nurse – Lower limb wound Care Team

Join Our Community Nursing Network

Locala is welcoming applications from enthusiastic, motivated, and caring Registered Nurses with an interest in tissue viability and wound care to join our Community Nursing Network (CNN) Lower Limb wound Care Team . Whether you are newly qualified or experienced, we will support you to develop within this specialist role.

Our service operates across Kirklees, working closely with nine Community Nursing Network teams to deliver high\-quality, person\-centred care to patients in clinic settings and in their own homes. As a Band 5 Community Staff Nurse within Lower Limb Wound Care Team , you will play a key role in assessing, planning, and delivering specialist wound care.

Your responsibilities will include:

  • Delivering high\-quality nursing care to patients in clinics and home environments.

  • Undertaking holistic assessments of patients with Lower Limb wounds.

  • Using Doppler ultrasound techniques and clinical interpretation skills to support diagnosis and care planning.

  • Contributing to Complex Wound and Leg Ulcer Clinics across the service.

  • Coordinating care in collaboration with Community Nursing Network teams.

  • Supporting patient self\-care, long\-term condition management, and health promotion initiatives.

  • Contributing to innovative projects that enhance patient outcomes and service delivery.
What we’re looking for:
  • A Registered Nurse with enthusiasm, motivation, and a keen interest in Wound Care.

  • A commitment to delivering high standards of compassionate, patient\-centred care.

  • Excellent communication and interpersonal skills.

  • The ability to work independently and as part of a multidisciplinary team.

  • Adaptability, reliability, and a proactive approach to care delivery.
Essential requirements:
  • A full driving licence and access to a vehicle, as the role involves travel across Kirklees to attend clinics and visit patients at home.
