The Role
As a Care Coordinator, you will play a vital role in ensuring the smooth and effective delivery of care services across the community. You will be responsible for coordinating care staff rotas, supporting the care team, and maintaining excellent communication with clients, families, and healthcare professionals.
This is a fast\-paced and rewarding role that requires strong organisational skills, problem\-solving ability, and a genuine passion for delivering outstanding care.
The successful candidate will also participate in the company’s out\-of\-hours on\-call service on a scheduled rota basis.
Key Responsibilities
- Coordinate and manage care staff rotas and schedules efficiently
- Ensure all care visits are allocated and covered appropriately
- Liaise with clients, families, care staff, and external professionals
- Respond promptly to changes in care needs and staffing requirements
- Support the recruitment, onboarding, and supervision of care staff
- Maintain accurate records and ensure compliance with CIW regulations and company policies
- Monitor service delivery to maintain high standards of care
- Participate in the out\-of\-hours on\-call rota as required
- Build and maintain positive working relationships with clients and the wider healthcare community
- Previous experience in a care coordination, scheduling, or domiciliary care role
- Strong organisational and time management skills
- Excellent communication and interpersonal abilities
- Ability to work calmly under pressure and manage changing priorities
- Good IT skills and experience using care management systems
- Understanding of CIW regulations and person\-centred care practices
- A compassionate, professional, and proactive approach
- Full UK driving licence preferred
