NHS Lothian is excited to be recruiting a Throughcare and Aftercare Nurse Practitioner to identify, assess and plan for the health needs of formerly looked after and accommodated young people in various settings. The primary role of this post is to improve health outcomes for this group of vulnerable young people with a focus on multi\-agency working and ensuring structures are in place to plan, manage and monitor the delivery of healthcare. There is a specific emphasis on the period of transition from childhood to adulthood and the services that support vulnerable young people through this life stage.
