Weigh To Go Outreach Nurse - Youth Health Service

The role:
We are looking for a motivated and compassionate Outreach Nurse to join our Weigh to Go service within NHS Greater Glasgow and Clyde. You will play a vital role in supporting young people aged 1218 to achieve healthier lifestyles through a structured weight management programme. Working across community settings, you’ll deliver person\-centred care, promote wellbeing, and empower young people to make positive changes.

You need to have a live NMC UK pin in order to be considered for this post.
All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.
Please note you must have a full UK driving license and access to a car for this role.
Duration, Location, and Working Pattern:
Duration: Permanent
Location: West House, Gartnavel Royal Hospital
Hours: 4\.5 Hours (Part Time)
Working Pattern: Evening work 18:00\- 21:30 (Wednesdays, Shettleston Health Centre) with some daytime hours as agreed by the hiring manager.
Please note the salary is Pro Rata for part\-time hours.
Key responsibilities:
- Deliver individualised, clinically based weight management support for young people aged 1218
- Assess eligibility and develop, implement and evaluate care plans in line with evidence\-based practice
- Work collaboratively with commercial weight management partners and multi\-agency teams
- Undertake holistic assessments considering physical, emotional and social factors impacting health
- Provide health promotion, education and behaviour change support to individuals and groups
- Maintain accurate clinical records and ensure compliance with professional and organisational standards
- Build effective relationships with young people, families and partner agencies to support positive outcomes
- Registered General Nurse with relevant post\-registration experience and a live NMC pin.
- Experience in primary care or community\-based settings
- Proven ability to make sound clinical decisions and work autonomously within professional boundaries
- Experience of working with young people and understanding their health and wellbeing needs
- Knowledge of current legislation, including child protection and confidentiality
- Excellent communication, interpersonal and IT skills with the ability to deliver sensitive information
- Ability to prioritise workload, work flexibly and contribute effectively within a multi\-disciplinary team
