Ward Medicines Management Technician
Working Part Time: 34 hours per week
The successful candidate will participate in the provision and development of the Pharmacy service to all wards and departments in the Trust. Supporting the use of medicines on the wards in terms of recording medication histories, reuse of patients own drugs, initiating supplies and reducing medicines waste.
They will contribute to the efficient discharge of patients by ensuring they have the medicines they need when they
are discharged including receiving appropriate counselling and understand how to take them.
To pro\-actively support the delivery of pharmaceutical care on wards through supporting ward staff in managing medicines and medicines reconciliation with patients. This will involve the planning and prioritisation of own workload whilst working as part of a team to provide pharmaceutical care.
To assess the suitability of the use of patient’s own medication, ensuring patients consent to use their medication whilst in hospital and to arrange the efficient and prompt supply of medicines to the wards considering the most cost effective options.
To assess patients suitability for compliance aids, and refer patients with compliance issues to the appropriate health care professional.
To assist the transfer of medicines when patients are transferred between wards.
To ensure that patients receive an efficient and timely supply of medicines during their stay and on discharge from hospital.
To undertake patient education in accordance with pharmacy procedures to ensure maximum concordance of their treatment and document such activity within a care plan/patients records as appropriate.
The Trust’s services are provided from three locations:
- The Countess of Chester Hospital: providing 473 general and acute beds.
- Ellesmere Port Hospital: providing 60 beds as a rehabilitation, intermediate and outpatient facility.
- Tarporley War Memorial Hospital: a base for community services which serve the local rural population.
The Trust works collaboratively within the wider Cheshire and Merseyside Integrated Care System. Due to its location on the border with North Wales, the Trust also works closely with the Betsi Cadwaladr University Health Board.
The Trust treats patients from England and some parts of Wales. During 2024/2025, there were approximately 643,000 patient attendances (inpatient, A\&E, outpatient and diagnostic) ranging from a simple outpatient appointment to major cancer surgery
