Transitions Supported Living is an established provider (since 2015\) supporting young people aged 16–18 in care as they prepare for adulthood and greater independence.
We are committed to providing high\-quality support in line with the Supported Accommodation (England) Regulations 2023, with safeguarding, positive relationships and outcomes for young people at the centre of our practice.
We work closely with social workers, safeguarding professionals, police, education providers and other partner agencies to identify and respond to risk while helping young people develop the skills, confidence and stability they need for adulthood.

The Role:
We are looking for an experienced and motivated Team Leader to provide day\-to\-day leadership to our Support Mentors and help ensure young people receive consistent, safe and high\-quality support.
This is a hands\-on leadership role and is ideal for someone ready to take the next step from an experienced Support Worker/Support Mentor or Senior Support role. We will provide level 3 leadership training.
You will work closely with the Service Manager and wider management team, helping to coordinate the day\-to\-day running of the service while providing guidance, supervision and direction to Support Mentors. Your direct staff responsibility will be for Support Mentors, ensuring they understand expectations, maintain professional standards and provide consistent support to young people.
You will still have direct involvement with young people. This is not an office\-based management position. We want Team Leaders who know the young people, understand their plans and risks, can model excellent practice and are visible and approachable to both young people and staff.
Key Responsibilities:
- Provide day\-to\-day leadership, guidance and support to Support Mentors.
- Line manage and complete regular supervisions with allocated Support Mentors.
- Lead by example and promote a positive, accountable and supportive team culture.
- Coordinate the day\-to\-day delivery of support and ensure young people receive the support identified within their individual plans.
- Maintain oversight of young people's support plans, risk assessments, key\-work and independence work, ensuring these remain current and meaningful.
- Support staff to recognise, record, report and appropriately respond to safeguarding concerns and changes in risk.
- Escalate safeguarding concerns appropriately to the Service Manager/DSL in accordance with Transitions procedures.
- Support young people when they are distressed or experiencing difficulties, maintaining appropriate professional boundaries and a trauma\-informed approach.
- Encourage young people to develop practical independence skills and participate positively within their home and community.
- Support young people preparing for move\-on and greater independence.
- Review the quality of staff recording, daily notes, key\-work, reports and other documentation, providing guidance where standards need to improve.
- Support the management team with quality assurance and maintaining standards across the service.
- Ensure Support Mentors follow Transitions policies, procedures and professional expectations.
- Contribute to team meetings, planning meetings and discussions regarding young people's progress and support.
- Liaise professionally with social workers, education providers, police, health professionals and other agencies where appropriate to the role.
- Support effective communication between Support Mentors, Senior Support Mentors and management.
- Identify areas where staff may require additional guidance, training or development and raise these with management.
- Address day\-to\-day practice or performance concerns with Support Mentors and escalate more significant concerns appropriately.
- Assist with maintaining safe and appropriate staffing arrangements and communicate any staffing concerns promptly.
- Promote positive outcomes, stability and independence for young people.
- Support house meetings, activities and opportunities for young people to have their voices heard.
- Maintain confidentiality and appropriate information\-sharing at all times.
- Undertake other reasonable responsibilities appropriate to the Team Leader role.
We are looking for someone who can lead from the front.
You need to be confident enough to challenge poor practice, supportive enough that staff feel able to come to you for guidance, and experienced enough to recognise when something needs to be escalated to senior management.
Most importantly, you need to genuinely enjoy working with young people.
Essential Requirements:
- Experience working with young people, preferably within supported accommodation, leaving care, residential or a similar setting.
- Experience of taking additional responsibility within a staff team or the ability to demonstrate clear leadership skills.
- Good understanding of safeguarding and managing risk involving young people.
- Ability to supervise, guide and support Support Mentors.
- Confidence addressing practice concerns and having professional conversations with staff.
- Good understanding of support planning, risk assessment and professional recording.
- Ability to communicate effectively with social workers and other professionals.
- Strong professional boundaries and the ability to remain calm when situations become challenging.
- Good written and verbal communication skills.
- Ability to organise and prioritise workload and maintain oversight of delegated tasks.
- Full UK driving licence with appropriate business\-use insurance.
- Willingness to undertake further training and professional development.
