Support Worker/Senior Support Worker

We are seeking compassionate and dedicated Support Workers to join our team in and around Taunton, Frome, Glastonbury, Ilminster, etc and surrounding area .

The successful candidate will provide essential support to patients in care homes or home care settings, ensuring their comfort, safety, and well\-being. Duties
Assist patients with daily activities such as bathing, dressing, and mobility
Support the development and implementation of personalised care plans
Monitor patients' health and report any concerns to senior staff
Help with medication administration as directed by healthcare professionals
Maintain accurate documentation of care provided using IT systems
Provide companionship and emotional support to patients
Assist with household tasks in care homes or home environments to promote independence
Follow health and safety guidelines diligently, including infection control procedures
Communicate effectively with patients, families, and healthcare team members to ensure coordinated care

Qualifications
Previous experience working in a care home or providing home care is advantageous but not essential; training will be provided
Valid driving licence is desirable for roles involving home visits
Good command of English, both written and spoken
Basic IT skills for maintaining accurate records and updating care plans
Excellent communication skills with the ability to listen empathetically and convey information clearly
Compassionate attitude with a strong commitment to patient welfare
Ability to work independently and as part of a multidisciplinary team
