Are you an experienced professional passionate about working with individuals with high needs and risks? ReconnectNow is launching a new 24\-hour supported living service, and we are building a dedicated and skilled team from the ground up to provide outstanding care and support.
About the Role:
We are preparing to welcome our first client, who has complex needs, and we require exceptional individuals with experience in:
- Supported living services
- High\-risk or secure environments
- Trauma\-informed or mental health\-focused care
What We’re Looking For:
- Proven experience working with individuals with high needs and risks
- Evidence of up\-to\-date training in health and social care (e.g., safeguarding, mental health, first aid, etc.)
- Relevant qualifications in health and social care or related fields
- A compassionate, professional, and adaptable approach
- Ability to manage challenging situations with empathy and skill
- A commitment to maintaining a safe and supportive environment

3\. JOB PURPOSE
We seek a dedicated and qualified Mental Health Support Worker experienced in supporting people with complex mental health needs to join our supported living team. In this role, you'll provide support to adults in their home within their community, this will involve but not limited to lone working and working with a multidisciplinary team.
The role includes promoting social inclusion by supporting clients to engage in daily activities that they have planned. This will include practical support around some daily living activities and wellbeing checks.
You will collaborate with other health professionals and families to provide person\-centred support and strategies that best meets the needs of a client.
Upholding ethical standards, you'll contribute to the holistic support of each client, traveling within the community as needed. Your role is vital in creating a safe space for self\-discovery and positive impact.

6\. WORKING CONDITIONS
Carry out duties and responsibilities effectively in a range of setting and operating.
The post holder may be working in environments where there is a risk of physical and verbal aggression.
The post holder must be able to work independently but will be accountable to, and supported by, the Manager.
The post holder will note their working week will be spread across Sunday to Saturday, based on a service needs led rota including sleep\-in shifts.
Occasionally, flexibility is required to participate in meetings, training and to support service needs.
The post holder will be required to lone work.
The post holder will be required to undertake escorting clients to appointments and other locations within the community.
7\. REHABILITATION OF OFFENDERS
Due to the nature of the work for which you are applying, the post is exempt for the provisions of Section 4(2\) of the Rehabilitation of Offenders Act 1974, by virtue of the Rehabilitation of Offenders Act 1974 (Exemptions Order) 1975\.
Applicants are therefore not entitled to withhold information about cautions and convictions and, in the event of employment, any failure to disclose such cautions or convictions could result in disciplinary action or dismissal by ReconnectNow Ltd. Any information given will be completely confidential and will be considered only in relation to an application for a position to which the order is applied.

Information derived from client interactions, staff details, and stakeholder communications is indispensable for the organisation's smooth operation and optimal utilisation of resources to benefit clients, stakeholders, and staff.
The responsibility for maintaining good data quality is a shared commitment among all individuals involved in recording client, staff, and other stakeholder information, whether their role is clinical or operational within ReconnectNow Ltd.
Every staff member engaged in recording client, staff, and stakeholder information, be it through electronic means or on paper, carries the responsibility to ensure that the data is accurate, timely, and as comprehensive as possible.
This commitment to data quality is crucial for the organisation's overall efficiency and its ability to serve clients, stakeholders, staff, and communities effectively.
10\. CONFIDENTIALITY
The post holder must maintain the confidentiality of information about clients, staff, other stakeholders, and the organisation business in accordance with the Caldicott principles and with legal obligations under the Data Protection Act 2018 (the ‘2018 Act’) and the EU General Data Protection Regulation (‘GDPR’) in respect of data privacy and security.

The post holder should also work within the boundaries described in ReconnectNow Ltd Information Governance Policy and all other related data protection policies.

11\. HEALTH AND SAFETY
Support Workers are required to be mindful of their responsibilities under the Health \& Safety at Work Act (1974\). It is essential that they diligently execute the agreed\-upon safety procedures to uphold a secure environment.
This commitment to safety is paramount in fostering a workplace that prioritises the well\-being of everyone involved.
Accidents involving employees and clients should be reported following established procedures and in adherence to the Health \& Safety at Work Act.
Immediately report any incidents, accidents, complaints, or other occurrences involving clients, staff and other stakeholders.
Strive to resolve issues whenever possible, provide accurate statements, and promptly report to the Manager.
Notify the Manager of any defects in equipment or the overall structure of the premises.
Initiate measures to evaluate risk management, aiming to minimise the potential impact on clients, other professionals, and all other stakeholders whenever feasible.
Consistently adhere to Infection Prevention \& Control policies, procedures, and best practice guidance to uphold elevated standards of infection prevention and control.
