An exciting opportunity has arisen for a Band 6 Occupational Therapist to work with Halton Intermediate Care and Frailty Service. We are looking for an experienced and well organised individual with a passion for promoting independence, driving a strength\-based approach and providing therapy interventions in a person's own home or discharge to assess unit in line with a home first approach.

We have been on an exciting journey to transform the intermediate care services within Halton and have brought together health and social care into a unified integrated team. Intermediate Care Unit

You will work as part of a large multidisciplinary team that includes: GP’s, Advanced Nurse Practitioners, Physiotherapists, Occupational Therapists, Social Workers, Therapy and Nursing Assistants, Dietician, Care Support Workers and Administrators.

The HICAF Service operates a 7 day service, currently covering the hours of 8am to 8pm, including bank holidays and weekends. You will be expected to work shifts to cover some of the weekends and bank holiday.

Our patients are at the heart of everything we do! We look for people who are committed to being part of a health and social care team that delivers excellent care and support to our patients. To help us achieve this, we expect you to hold responsibility for your own caseload, perform patient therapy assessments, provide a diagnosis, develop, implement and evaluate individualised holistic treatment programmes, and deliver a high standard of evidence\-based care.

In all areas, you will work in a multi\-disciplinary team, follow appropriate pathways of care, liaising with and referring to other agencies as appropriate.

We expect you to be hardworking, motivated with excellent clinical, communication, organisational and time management skills. A flexible approach to day\-to\-day tasks is essential, in order to meet the demands of the service. You will also be expected to travel across Halton, in line with service needs, so access to transport is essential

As a Band 6 Occupational Therapist, you will contribute to the clinical supervision of students and junior staff. You will participate in service improvement projects, audits, in service training, and work collaboratively with our hospital and community colleagues to ensure our patients receive outstanding care.
