Social Worker - Adults Discharge to Assess
<p><span data-olk-copy-source="MessageBody">Liquid Personnel is recruiting Social Worker for its client's Adults Discharge to Assess Team at Bristol. This is a fantastic opportunity to support citizens during a critical stage in their care journey, ensuring safe and effective transitions from hospital to community settings.<br></span></p>
<p><strong>What will your responsibilities be?</strong></p>
<ul>
<li>Assess and support citizens on discharge from hospital across three discharge pathways</li>
<li>Complete post-discharge assessments and reviews in line with the Care Act</li>
<li>Undertake Care Act Assessments (CAA) and support planning</li>
<li>Carry out Best Interest (BI) decisions and work within the Mental Capacity Act (MCA)</li>
<li>Manage safeguarding concerns in line with statutory responsibilities</li>
<li>Provide duty cover where required</li>
<li>Attend Quality Assurance (QA) panels to support funding decisions</li>
</ul>

<p><strong>Qualifications & Experience:</strong></p>
<p>To be successful in this role you must have, </p>
<ul>
<li>Qualified Social Worker with relevant Social Work England registration</li>
<li>Experience within Adults Services, ideally discharge or hospital-based teams</li>
<li>Strong knowledge of Care Act, MCA, and safeguarding legislation</li>
<li>Ability to work in a fast-paced environment and manage competing priorities</li>
</ul>
