Senior Social Worker - Transitions Team (0-25 Service)
Are you a children's or adults social worker who enjoys the bridge between the two, supporting young people into adulthood? Pro Social Partners are working in partnership with Medway Council to recruit a Senior Social Worker into their Transitions Team. This is a permanent role in one of the most rewarding areas of practice.
About the team and the role
The Transitions Team walks alongside young people aged 14 to 25 as they move from childrens services into adult social care, which is a big moment in their lives. Day to day you will:
- Hold a caseload of young people preparing for adulthood
- Carry out assessments under the Care Act 2014 and the Children Act 1989
- Plan transitions around education, health, housing and independence
- Work across childrens and adults teams and with health partners
This is a permanent senior role, so you can settle and build genuine relationships with the young people and families you support. Transitions is a growing and highly valued specialism that sits across childrens and adults social care, so it broadens your practice and your career options. - Registered with Social Work England
- A recognised social work qualification
- Experience with young people with disabilities or complex needs
- Confident working across childrens and adults legislation
- Salary of £47,435 per year
- A permanent senior role with a supportive Kent authority
