The role:

We are looking for enthusiastic and motivated health care support workers to join our developing Hospital at Home service for Renfrewshire and East Renfrewshire. The role involves supporting frail patients experiencing acute changes, working within a multidisciplinary team (MDT), and delivering acutelevel care in the person’s home or homely environment

NHS Greater Glasgow and Clyde (NHSGGC) may be able to provide visa sponsorship for certain roles, subject to the post meeting the relevant eligibility criteria. Sponsorship is not guaranteed and will be determined on a case\-by\-case basis following interview and during the pre\-employment verification process. Duration, Location, and Working Pattern:

  • Base location: Royal Alexandra Hospital

  • Various hours available, Permanent

  • Work delivered across Renfrewshire HSCP and parts of East Renfrewshire.

  • Shift pattern: 7 days a week, 8 am8 pm., including public holidays
Key responsibilities:
  • Supporting admission of frail patients to the Hospital at Home virtual ward.

  • Delivering acutelevel interventions in the patient’s home or homely environment.

  • Working under the direction and supervision of the registered MDT.

  • Collaborating closely with acute teams, including ED, MAU, AMU, and the Older Adults Assessment Unit.

  • Contributing to safe, highquality care and supporting patient flow. Learn more about MDT responsibilities or acute care at home.
Knowledge, training, qualifications and/or experience required to do the job:
  • Relevant experience in community or acute settings is required.

  • Understanding of frailty pathways and acute care delivery in nonhospital environments is beneficial.
