Senior Clinical Fellow Community Frailty
This post is fixed term/secondment for 24 months due to meet the needs of the service
The successful candidate will join the Community Hospital Service based at Brecon Hospital, working within a supportive multidisciplinary team providing care for patients with frailty, rehabilitation and complex medical needs. The post offers an excellent opportunity to develop skills in community\-based medicine, integrated care and the management of patients transitioning from acute hospital settings.
The post holder will work under the supervision of the Consultant Physician/ Specialist and will be supported to develop clinical, leadership and quality improvement skills. In keeping with Powys Teaching Health Board's integrated approach to care, there may also be opportunities to gain experience within Mental Health and Learning Disability services, depending on individual interests and service requirements, providing exposure to a broader range of community and integrated care pathways.
