PRACTICE NURSE – JOB DESCRIPTION

Position: Practice Nurse
Hours: 21 hours per week
Location: George Eliot Medical Centre
Responsible to: GP Partners / Practice Manager
Salary: Dependent on experience and qualifications
Contract: Permanent

This is not a training post. The successful applicant must be able to undertake the required Practice Nurse duties independently from commencement, including cervical cytology, childhood and baby immunisations, long\-term condition reviews/QOF, cytology

Job Summary

We are seeking an experienced and motivated Practice Nurse to join our friendly and supportive GP practice for 21 hours per week.

The successful candidate will be an autonomous and competent Practice Nurse who is able to provide a broad range of general practice nursing services. You will work as part of the wider practice team to deliver high\-quality, safe and effective patient care, supporting the practice to achieve its clinical, QOF and contractual requirements.

The role will suit a nurse who is confident working independently, has excellent clinical and communication skills, and is committed to delivering a high standard of patient\-centred care.

Key ResponsibilitiesLong\-Term Condition Management

  • Undertake structured reviews and ongoing management of patients with long\-term conditions.

  • Carry out appropriate clinical assessments, observations and investigations.

  • Provide appropriate advice, education and self\-management support.

  • Support the achievement of relevant QOF indicators and clinical targets.

  • Identify patients requiring further clinical review or escalation and refer appropriately.

  • Maintain accurate and contemporaneous patient records.
Long\-term condition work may include, but is not limited to:
  • Diabetes

  • Asthma

  • COPD

  • Hypertension

  • Other relevant long\-term conditions
QOF and Practice Performance
  • Support the practice in meeting its QOF and other contractual requirements.

  • Maintain accurate coding and documentation.

  • Identify patients who require recall, review, investigation or follow\-up.

  • Work with the clinical and administrative teams to maximise appropriate patient care and achievement.
Cervical Screening / Cytology
  • Undertake cervical screening (smears) in accordance with national guidance and professional standards.

  • Maintain appropriate knowledge and competence in cervical screening.

  • Ensure appropriate documentation and coding of results.

  • Contribute to the practice cytology failsafe system, ensuring abnormal and outstanding results are appropriately followed up.

  • Work closely with the practice team to ensure patients requiring further investigation are appropriately identified and followed up.
Childhood Immunisations
  • Administer childhood and baby immunisations in accordance with the national vaccination schedule and current guidance.

  • Assess patients prior to vaccination and identify any contraindications or concerns.

  • Provide parents/carers with appropriate information and advice.

  • Maintain accurate vaccination records and coding.

  • Support vaccination uptake and recall systems.
Adult Immunisations and Vaccinations
  • Administer routine adult vaccinations and immunisations within the nurse's scope of competence.

  • Support seasonal vaccination programmes and other practice vaccination campaigns.

  • Ensure appropriate documentation and coding.
Mental Health Injections
  • Administer long\-acting injectable medication for mental health patients where appropriately trained and competent.

  • Undertake relevant observations and monitoring in accordance with the patient's care plan and local protocols.

  • Liaise with the appropriate mental health team and clinicians where concerns are identified.

  • Maintain accurate and timely records.
Infection Prevention and Control
  • Support the Practice in maintaining high standards of infection prevention and control.

  • Participate in infection control audits and monitoring.

  • Follow practice infection control policies, procedures and national guidance.

  • Ensure appropriate cleaning, decontamination and clinical waste procedures are followed.

  • Identify and report infection control risks or concerns.

  • Contribute to maintaining safe and hygienic clinical environments.
General Practice Nursing
  • Undertake routine nursing assessments and clinical procedures within the nurse's competence.

  • Carry out observations, blood pressure checks, pulse, respiratory rate, oxygen saturation and other appropriate assessments.

  • Undertake blood tests and other investigations where trained and competent.

  • Provide wound care and appropriate treatment.

  • Provide health promotion and lifestyle advice.

  • Promote preventative healthcare and patient self\-management.

  • Recognise deterioration and appropriately escalate concerns to the GP or relevant clinician.

  • Work collaboratively with GPs, ANPs, healthcare assistants, pharmacists and the wider practice team.
Clinical Governance and Quality
  • Work within the NMC Code of Professional Conduct and maintain professional registration.

  • Maintain clinical competence and undertake appropriate continuing professional development.

  • Participate in clinical audits and quality improvement activities.

  • Contribute to significant event reviews, learning and service improvement.

  • Maintain patient confidentiality and comply with information governance requirements.

  • Follow practice policies, protocols and standard operating procedures.

  • Maintain appropriate safeguarding knowledge and responsibilities.

  • Work within the practice's health and safety, risk management and incident reporting procedures.
Communication and Team Working
  • Develop effective working relationships with patients, carers and colleagues.

  • Communicate clearly and sensitively with patients and their families.

  • Work effectively as part of the multidisciplinary practice team.

  • Liaise with external healthcare professionals and community services where appropriate.

  • Contribute to practice meetings and clinical discussions when required.
Professional Responsibilities

The post holder will:

  • Maintain current NMC registration.

  • Work within their professional scope of practice and competence.

  • Maintain mandatory and role\-specific training.

  • Maintain appropriate clinical knowledge and skills through CPD.

  • Participate in appraisal and performance review.

  • Maintain patient confidentiality at all times.

  • Comply with practice policies and procedures.

  • Undertake other reasonable duties appropriate to the role and level of responsibility.
Person SpecificationEssential
  • Registered Nurse with current NMC registration.

  • Relevant experience working within general practice or primary care.

  • Experience in long\-term condition management.

  • Competent and experienced in cervical screening/cytology.

  • Competent in childhood and baby immunisations.

  • Experience of administering vaccinations and injections.

  • Ability to undertake QOF\-related clinical work.

  • Good understanding of infection prevention and control.

  • Excellent communication and interpersonal skills.

  • Ability to work autonomously and as part of a multidisciplinary team.

  • Good clinical assessment and decision\-making skills.

  • Commitment to patient\-centred care.

  • Ability to maintain accurate clinical records.
Working Arrangements

The Practice Nurse will work 21 hours per week, with working days and session times agreed with the practice. There is flexibility on days and timings.

The successful candidate will be expected to work flexibly as required by the needs of the practice and patient population.

Job Type: Part\-time

Pay: £18\.00\-£23\.00 per hour
