PCN Care Coordinator
Overview
This role reflects the collaboration between three General Practices which create one Primary Care Network (PCN) to streamline processes, improve quality of Enhanced Services and increase engagement in vulnerable patient populations.
You will work as part of a team to build a holistic approach in implementing new processes and strengthening current practices to support the practice teams in areas such as safeguarding, end of life care / palliative care, cancer support and screening, adult learning disability and serious mental illness annual reviews, as well as other key NHS England Health Promotions. Working with clinicians, practice managers and non\-clinical staff, this role will seek to meet key performance indicators set out by the PCN.
Experience / person specification
· An applicant for this role will be able to demonstrate excellent formal writing skills, as well as competent skills within Microsoft Excel and Word
· Time management and communication are key for team working, both within the PCN and the wider community teams
· Previous working experience working within General Practice or the Health and Social Care Sector is preferable to demonstrate a solid understanding of confidentiality and information sharing policies
· Training on relevant policies and legal frameworks will be provided to develop core competencies
· Organisational – (Diary keeping, reminders, MDT’s)
· Communication – (written, verbal, across different disciplines)
Job details
· Full time position \- 37\.5 hours per week
· NHS Pension
· 25 days paid holiday per annum (\+ bank holidays)

Pay: £14\.00\-£15\.90 per hour
