If you’re an Occupational Therapist who thrives on fast‑paced, person‑centred work and wants to make a tangible difference to people living in York, this role of Discharge to Assess (D2A) OT could be the perfect next step.
The Opportunity
You’ll use your clinical assessment skills and a strength‑based approach to support adults with a wide range of physical conditions. You will enable safe, timely support for people who have been discharged from hospital to a short stay unit. Working directly in the short stay unit, and within people’s homes, you’ll complete holistic assessments, develop tailored interventions, and review progress to ensure the best possible outcomes.
You’ll work autonomously day‑to‑day, with support from the wider social care team and will receive clinical supervision from an Occupational Therapist, ensuring you always have professional guidance when you need it. You’ll also have access to OT‑specific CPD events delivered by the Council.

What You’ll Be Doing
Working with social workers and the wider MDT to develop person\-centred support, you will be conducting functional assessments across a broad range of physical and medical conditions, this includes moving and handling assessments.
This role is suitable as a job share opportunity.
As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C2: Can express themselves spontaneously at length with a natural conversational flow, avoiding or backtracking around any difficulty so smoothly that the person with whom they are conversing is hardly aware of it.
Hybrid working arrangements are available for this role. Each role at CYC is different and your working arrangements will be determined based on the requirements of the role and in collaboration with you and your needs as part of the recruitment process. Your contractual location will be the designated council office for your team.
