Band 5 Occupational Therapist \- Community Rehabilitation Service (CRS), South Somerset

Empower patients to live life to the fullest, supporting them to remain independent and focusing on what truly matters to each individual.

Work at the heart of the community, collaborating closely with Rapid Response, D2A, acute and community services, and a wide network of partners including GPs, PCNs, ASC, DNs, and OPMHT.

Main duties, tasks \& skills required

You will be expected to carry out specific therapy assessments which will include goal setting and treatment planning, assessing risk, promoting and enabling independent functioning. You will be committed to a person\-centred approach to your work, have an ability to prioritise and work flexibly. Working alongside our provider teams and rehabilitation practitioners.

Are you a fully qualified Occupational Therapist with HCPC registration (or pending if you're still studying)?

Do you have a desire to enhance your post\-graduate training and experience?

Do you want to get involved in a pioneering service that welcomes development initiatives and integration projects?

Keen to take an active role in developing and nurturing others, both personally and professionally?
