In partnership with Macmillan Cancer Support, we are recruiting a Macmillan Cancer Social Prescriber (Band 4\) to support delivery of the Improving my Cancer Journey (ICJ) service within the Primary, Community and Intermediate Care (PCIC) Directorate across Cardiff and the Vale.

This role supports people affected by cancer through a non‑clinical, community‑based social prescribing approach, working primarily within primary care and community settings.

The postholder will act as a friendly point of contact from referral, holding personalised conversations to understand what matters most to people and supporting them to access community, voluntary sector and wellbeing services alongside their cancer care. Approved ICJ conversation tools will be used in line with agreed protocols to support consistent, high‑quality non‑clinical discussions.

Working as part of the wider ICJ team and under the supervision of the Macmillan Project Manager, the postholder will work closely with primary care, cancer teams, community organisations and third sector partners. The role focuses on information, signposting and navigation, with clear professional boundaries and appropriate escalation when required.

Welsh language skills are desirable; Welsh and/or English speakers are welcome to apply. This post is fixed term due to Macmillan funding, with potential for extension subject to future funding. Secondment applicants must obtain line manager approval prior to application.

Act as a member of the Improving my Cancer Journey (ICJ) team within Primary, Community and Intermediate Care (PCIC), supporting people affected by cancer through a non‑clinical, person‑centred approach.

Act as a friendly and accessible point of contact for people referred to the service, providing support through face‑to‑face, telephone and virtual contact.

Hold structured, personalised conversations to understand what matters most to people, focusing on non‑clinical practical, emotional and social needs.

Use approved ICJ conversation tools in line with agreed protocols to support high‑quality non‑clinical discussions and appropriate signposting.

Provide clear information and signposting to community, voluntary sector and wellbeing services, supporting individuals to navigate available resources.

Maintain appropriate follow‑up contact to support engagement, recognise changes in circumstances, and escalate concerns when outside the scope of the role.

Build and maintain effective working relationships with primary care, community and third sector partners to support joined‑up, person‑centred pathways.

Accurately record interactions and outcomes using approved digital systems, contributing to service monitoring, evaluation and improvement.
