Specialist Nurse – Hospice at Home
Location – Nr Stroud
Salary – £20\.35 hourly rate
Hours – Part Time, 15 hours per week (Rota\-based, 7 days a week, 7am – 3pm \& 2pm 10pm)
About the Role
Are you an experienced Registered Nurse passionate about delivering outstanding end‑of‑life care in the community? We’re looking for a compassionate and motivated Specialist Nurse to join our Hospice at Home team.
In this rewarding role, you will:
- Lead and support our Hospice at Home team to deliver high‑quality care in patients’ homes
- Manage referrals, coordinate care, and work closely with multidisciplinary partners
- Provide expert clinical leadership, assessment, and guidance to staff, patients, and families
- Help prevent unnecessary hospital admissions, enabling patients to be cared for at home
- Support service development, governance, staff training, and clinical standards
- Line‑manage HCA and RN teams, providing supervision, mentorship, and performance reviews
- Registered Nurse with NMC PIN
- Significant experience at Band 5 or equivalent
- Confident managing staff and clinical priorities
- Excellent communication and organisational skills
- Experience in community care and supporting families
- Competent using electronic patient records
