Responsibilities

• Assisting to access the community.


• Support the development and implementation of personalized care plans tailored to individual needs


• Monitor health status, report changes, and document observations accurately using IT systems


• Provide companionship and emotional support to promote mental well\-being


• Facilitate communication between patients, families, and healthcare professionals to ensure coordinated care


• Drive or accompany patients to appointments or outings as required, ensuring safety at all times


• Maintain a clean, safe, and welcoming environment in accordance with health and safety regulationsRequirements


• Valid driving license is highly desirable for roles involving transportation of patients or residents


• Strong communication skills in English to effectively interact with patients, families, and team members


• Basic IT skills for documenting care activities and updating electronic health records accurately


• Ability to follow detailed care plans and adapt to changing needs of individuals under your supervision


• Compassionate attitude with a genuine desire to support others’ health and well\-being


• Good physical fitness to assist with mobility tasks and long shifts

Pay: From £13\.50 per hour
