• the support of the practice nursing team in the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the patient population.


• The healthcare assistant will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education.

• Assist registered nurses and other healthcare professionals in implementing personalized care plans tailored to each patient’s needs and Supporting the practice nurse with health promotion programmes


• Monitor and record vital signs, health status updates, and any changes in patient conditions accurately using IT systems.


• Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, recording findings accurately


• Facilitate routine and 24\-hour BP monitoring, advising patients accordingly


• Undertake wound care, dressings and other clinical tasks as required


• Undertake new patient health checks


• Communicate effectively with patients, families, and team members to ensure clear understanding of care procedures and patient needs.


• Maintain a clean, safe, and organized environment by adhering to infection control protocols and organizational standards.


• Assist with administrative tasks such as updating patient records, scheduling appointments, and managing documentation efficiently.


• Participate in ongoing training to stay current with best practices in nursing support and healthcare technology.


• Support the practice nurse with the management of chronic disease clinics


• When trained, administer flu vaccinations


• Ensure specimens are recorded and ready for onward transportation


• Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the practice IPC policy


• Deliver opportunistic health promotion where appropriate

• Previous experience in Primary care in a HCA role is preferable


• Experience with system one is preferable but not essential


• Ability to develop and follow detailed care plans with precision and compassion.


• Strong communication skills to effectively interact with patients, families, and multidisciplinary teams.


• Excellent organizational skills to manage multiple responsibilities efficiently while maintaining attention to detail.
