Responsibilities

• Maintain clear and positive communication with clients, families, and healthcare professionals to ensure coordinated care


• Drive or accompany clients on errands or appointments when required, ensuring safe transportation


• Keep accurate records of care activities using IT systems and document any changes in condition or concerns


• Promote a warm, respectful atmosphere that encourages independence and dignity for all individuals

Skills

• Valid driving license and ability to drive safely for home care or outings


• Experience working in a care home or providing home care services is preferred


• Knowledge of creating and following care plans tailored to individual needs


• Strong communication skills in English to effectively interact with clients, families, and team members


• Proficiency with IT systems for documentation and reporting purposes


• Excellent interpersonal skills with the ability to listen actively and convey information clearly


• Ability to work independently as well as part of a multidisciplinary team
