Summary
We are seeking a dedicated Frailty Healthcare Assistant to join our team in providing exceptional support to our elderly and vulnerable patients.
The ideal candidate will have excellent IT, administrative and communication skills. Previous experience with Systmone is desirable but not essential.
You will project a positive and professional image to patients and other visitors, either in person or on the telephone.
This role would suit someone who has either community nursing experience or has held a post in hospital or social care. The role will involve visiting patients in their homes so you will need to have a clean driving licence and access to a car that you can use for work purposes.
The role will involve co\-ordination of clinical case management for frail patients and those with long\-term conditions who are at risk of further deterioration, an avoidable admission or unnecessary length of hospital stay. The focus will be on supporting frail patients to remain living in their own homes. You will work closely with GPs, Practice and District Nurses and Community and Social Care teams and will be supported by the team at Frithwood Surgery.
