Job Summary – Community Support Worker (Thumbs Up Care)
We are seeking a compassionate and reliable Community Support Worker to join Thumbs Up Care, supporting adults with learning disabilities, autism, mental health needs and complex presentations in their own homes and local communities. The role focuses on promoting independence, dignity and choice while delivering high‑quality, person‑centred support.
You will assist individuals with a wide range of daily living tasks, including personal care, medication, meal preparation, community access, attending appointments, and supporting people to engage in meaningful activities. This is a rewarding role where you will make a genuine difference to people’s lives by helping them build confidence, develop skills and participate fully in their communities.
Responsibilities
- Provide personalised support to people in their own homes and during community activities
- Assist with daily living tasks such as personal care, dressing, hygiene, and mobility support
- Administer and prompt medication in line with organisational policies and MAR guidance
- Support individuals to attend medical appointments, reviews, and health‑related activities
- Encourage and assist people to access community opportunities such as shopping, social groups, leisure activities, volunteering and education
- Promote independence by supporting skills such as cooking, cleaning, budgeting and household tasks
- Prepare meals and support individuals with nutritional needs and dietary requirements
- Maintain a safe, clean and supportive home environment
- Provide emotional reassurance, positive behaviour support and person‑centred communication
- Accurately record daily notes, medication records, incident reports and other documentation
- Work collaboratively with families, professionals and the wider support team
- Drive individuals to appointments or community activities where required (if a driver)
- Respond appropriately to emergencies and follow safeguarding and health \& safety procedures
- Experience in supported living, community support or domiciliary care is desirable
- Understanding of learning disabilities, autism, mental health needs and positive behaviour support
- Skills in personal care, medication administration, daily living support and communication
- Ability to support individuals with mobility needs, including use of aids and equipment
- A valid driving licence is desirable for community‑based support
- Compassionate, patient and respectful approach to care
- Strong communication skills and ability to build positive relationships
- Ability to work independently and as part of a team
- Flexibility to work a range of shifts including mornings, evenings and weekends
