Community Support Worker
N\-Able Services Ltd is a case management company that assists individuals in accessing the care, support, rehabilitation that they require. They act as an agent for the individual and act as a professional guide and line manager. The appointed person will be employed by the Client through their Financial Deputy acting on their behalf.
We are seeking an experienced and active individual to act as a support worker to our Client in the Preston area. Our Client has a traumatic brain injury. He has physical, cognitive, and speech and language difficulties. He continues to compete in adapted sports at a high level and will require an individual who can participate alongside him. Additionally he requires support in daily living and maintenance of therapy goals.

Principal Responsibilities
- To develop a trusting relationship with the Client and to facilitate their independence and quality of life as far as possible.
- To encourage, support and assist the Client to build on existing skills and develop new ones.
- To develop an appropriate rapport/trust with the Client’s family.
- To maintain relationships within the various placements that the client attends.
- To adhere to the Support / Care plan at all times, raising any concerns with the Case Manager as they arise.
- To work with the multidisciplinary team under the direction of the case manager in ensuring that treatment goals are adhered to and maintained.
- To report regularly to the Case Manager the progress of the Client in relation to the care and well being of the client.
- To always consider the Client’s health, safety and welfare when working with them.
- To travel with and accompany the Client on community activities as appropriate.
- To encourage the Client to make choices and decisions appropriate to their abilities.
- To promote a level of independence suitable to the Client’s abilities.
- To treat the Client as an individual with specific needs and wants, rights and responsibilities.
- To respect the privacy of the Client and their family.
- To respect Client confidentiality at all times within the framework of confidentiality policies.
- To be able to work individually and as part of a team.
- To be well motivated and demonstrate initiative.
- To liaise closely and communicate with the family, Case Manager and other professionals and agencies as required.
- To attend and participate in Client review meetings as requested by the Case Manager.
- To attend and complete any relevant training as agreed with the Case Manager.
- To accept support and advice from the Case Manager and receive regular supervision.
- To complete any other reasonable task within your abilities as directed by the Case Manager.
- To adhere to the scheduled rota/timetable and to immediately report any difficulties with its completion.
- To retain any appropriate receipts for reimbursement
- To maintain appropriate records as requested.
- To complete accurately daily reports/records.
- If appropriate, to assist in the administration and monitoring of prescribed medication.
- To report all accidents/incidents to the family and Case Manager and complete all required paperwork.
- To assist the Case Manager in relation to team management and supervision of staff as required
1\. To attend meetings held to discuss the Client’s progress and future plans when requested.
2\. To accept regular supervision from the Case Manager or designated person.
3\. To participate in mandatory training recommended as appropriate to the position.
4\. To accompany and to provide care and support for and to the Client on holidays and other journeys away from home.
Constraints
1\. No task must be undertaken unless the post holder is competent and qualified to carry out the task.

Pay: From £15\.52 per hour
