The Band 6 Crisis Team Practitioner plays a key role in delivering rapid, high\-quality mental health assessment and intervention for individuals experiencing acute psychological distress. Working within a multidisciplinary crisis or home treatment team, the post holder provides urgent, person\-centred care as an alternative to hospital admission wherever possible.

The role involves conducting comprehensive mental health and risk assessments, developing and implementing crisis care plans, and delivering short\-term therapeutic interventions. Practitioners work in fast\-paced environments, often responding to referrals from A\&E, community services, GPs, and other agencies, ensuring prompt and safe decision\-making. A core element of the role is effective risk management, including assessing risk of harm to self or others and implementing appropriate safeguarding measures. As a senior member of the team, the Band 6 practitioner provides clinical leadership, supporting junior staff, students, and support workers through supervision and guidance. They contribute to service improvement, audit, and quality initiatives, ensuring adherence to professional standards, policies, and evidence\-based practice. Strong communication and partnership working skills are essential, particularly when engaging with service users, families, and carers during times of crisis. The Band 6 Registered Professional within the Crisis Resolution and Home Treatment Team (CRHT) plays a vital role in delivering high\-quality, patient\-centred mental health care to individuals experiencing acute psychiatric crises. This role operates within a multidisciplinary, age\-independent service that provides 24/7 support, aiming to assess, treat, and stabilise individuals in the least restrictive environment, primarily within their own homes. The overarching objective is to promote recovery, prevent unnecessary hospital admissions, and facilitate early discharge from inpatient services wherever possible.

At its core, the role focuses on providing urgent mental health assessments, formulating care plans, delivering evidence\-based interventions, and supporting service users through periods of significant distress. The practitioner is responsible for conducting risk assessments, identifying immediate needs, and implementing therapeutic interventions tailored to individual circumstances. By working within patients’ home environments, the practitioner supports a more personalised and recovery\-focused approach to care.

The role also involves active participation in “gatekeeping” functions, ensuring that admissions to inpatient services are clinically justified and that alternatives such as home treatment are fully considered. Similarly, the practitioner contributes to facilitating early discharge by ensuring patients can be safely supported in the community.

The Band 6 practitioner is expected to apply advanced clinical knowledge and skills in assessing, planning, and delivering care. This includes evaluating complex patient information, identifying risks, and determining appropriate interventions. The practitioner collaborates closely with other professionals to ensure consistency in care and to address any discrepancies in assessments or treatment plans.

A significant component of the role is delivering therapeutic and evidence\-based interventions aligned with established care pathways. The practitioner also monitors patient progress, evaluates outcomes, and adjusts care plans as necessary. Ensuring patient safety is paramount, including identifying safeguarding concerns and taking appropriate action in accordance with organisational policies.

In addition, the practitioner promotes holistic care by addressing both mental and physical health needs, providing education and support to service users, families, and carers. This holistic approach recognises the interconnectedness of mental and physical wellbeing.

Effective communication is a central aspect of the role. The practitioner must engage with a wide range of stakeholders, including service users, carers, multidisciplinary team members, and external partners such as voluntary and community organisations. Communication often occurs in sensitive, complex, or emotionally charged situations, requiring tact, empathy, and professionalism.

The role also involves leading or contributing to multidisciplinary reviews, advocating for service users, and ensuring that care decisions are clearly documented and communicated. Strong written and verbal communication skills are essential for maintaining accurate records and ensuring continuity of care.

As a Band 6 professional, the post holder takes on leadership responsibilities within the team. This includes supervising junior staff, providing mentorship, and contributing to staff development. The practitioner may also act as a clinical resource, sharing knowledge and supporting service improvements.

The role requires participation in the operational duty system on a rotational basis. During these periods, the practitioner is responsible for managing workload distribution, skill mix, and resource allocation, ensuring that the team operates effectively and meets service demands. They may also deputise for the team manager when required.

Additionally, the practitioner is responsible for managing their own workload, prioritising tasks, and responding to changing demands within a fast\-paced and often unpredictable environment.

The practitioner must work within the limits of their professional competence and adhere to relevant codes of conduct and legal frameworks, such as the Mental Health Act and the Mental Capacity Act. They are responsible for maintaining high standards of professional practice, participating in supervision, and engaging in continuous professional development.

Maintaining confidentiality and complying with data protection regulations are critical aspects of the role. The practitioner must ensure accurate and timely documentation using both electronic and manual systems, adhering to information governance standards.

The role also includes contributing to audit, research, and service development activities, supporting the continuous improvement of care delivery.

The CRHT environment is dynamic and demanding, requiring the practitioner to work in a variety of settings, including service users’ homes and community locations. The role involves exposure to potentially challenging situations, such as dealing with distressed individuals, conflict, or unpredictable behaviour. Emotional resilience and the ability to maintain professionalism under pressure are essential.

The practitioner must also be prepared for agile working, adapting to service needs and managing frequent interruptions and competing priorities. Physical requirements include travelling between locations and occasionally handling equipment for patient care.

The role carries a strong responsibility for safeguarding children, young people, and vulnerable adults. The practitioner must remain vigilant, identify concerns, and follow appropriate safeguarding procedures.
