You will be joining friendly, supportive community nursing team who work closely together to meet the needs of our housebound population, delivering high‑quality, patient‑centred care and supporting people to remain safely in their own homes.

The successful applicants will be responsible for delivering high‑quality, evidence‑based nursing care to patients in their own homes, working as part of the South Hardwick Community Nursing Service.
This post is offered on a 12‑month fixed‑term basis to provide maternity leave cover, with working hours available at both South Normanton and Clay Cross.
This role supports patients across the community, including those accessing community support beds, with a focus on promoting recovery, independence, and a safe return to their own homes wherever possible. You will be expected to assess, plan, implement, and evaluate individualised care, encouraging self‑care while maintaining patient privacy, dignity, and choice.
An important aspect of the role includes providing advice, guidance, and support to junior staff, and actively supporting the District Nurse / Senior Community Nurse in maintaining a safe and effective clinical environment.
You will work closely with the wider multidisciplinary team, including Therapy Services, General Practice, and specialist services, contributing to coordinated, patient‑centred care across the pathway.
