Job summary

Join our friendly and enthusiastic Older Persons Community Mental Health Team as a Band 5 Mental Health Nurse. We support adults aged 65 and over experiencing moderate to severe mental illness, helping them live well in their own homes.

You will work within a multi\-disciplinary team alongside healthcare professionals, providing assessment, treatment, and support. This role offers the chance to develop your skills through Memory Assessments, Liaison Services, and supervision opportunities.

Main duties, tasks \& skills required

As a Mental Health Nurse, you will be responsible for key working a caseload of older adults with complex mental health needs.

Providing evidence\-based psychosocial interventions, contribute to risk management, and work closely with patients, carers, and colleagues across community and hospital settings.

Your role includes participating in team development, service improvement, and supporting junior staff. You will also play a vital part in safeguarding, care planning, and ensuring high standards of communication and record keeping.

Job description

This role involves delivering high\-quality patient care through assessment, planning, and therapeutic interventions using a recovery\-focused approach.

You will work flexibly across the community, including care homes, and liaise with other services such as the Intensive Dementia Service and Primary Care Networks. Responsibilities include managing risk, safeguarding adults, medication oversight, and participating in clinical governance and research. You will provide supervision to junior staff, contribute to service reviews, and support continuous professional development.
