Community Health and Wellbeing Worker

We are seeking three dedicated, empathetic, and culturally aware Community Health and Wellbeing Workers (CHWW) to join teams in general practice in Edmonton. Are you able to support people get the health and social services they need? If so, we want to hear from you.
Our area is rich in diversity, and we need someone who understands the unique language, cultural backgrounds, and challenges of residents in the Edmonton area.
Your Role \& Impact
As a CHWW, you will be the "bridge" between community members and formal health systems, helping to reduce health inequalities. Your day\-to\-day work will involve:
- Community Engagement: Conducting outreach to build trust, offering health education, and organizing community events.
- Case Navigation: Assisting residents in navigating health and social services, including scheduling appointments, arranging transportation, and explaining health plans.
- Health Promotion: Delivering culturally appropriate advice on chronic disease management, nutrition, maternal/child health, and mental well\-being.
- Advocacy \& Support: Empowering residents to address social determinants of health (housing, food security, immigration).
- Data Collection: Maintaining accurate, confidential records of client visits.
- Trusted Community Member: You must have a deep understanding of, and ties to, the diverse cultural and ethnic groups in Edmonton.
- Knowledge: Understanding of what can impact a person or community’s health
- Qualification: Willingness to obtain Level 3 Certificate in Community Health and Wellbeing
- Experience: Experience working in a community\-based setting, social service, or volunteer role supporting marginalized populations.
- Skills: Excellent communication, empathy, and active listening skills. Computer literacy for basic documentation.
· Ability to speak one or more of the following community languages:
Somali, Romanian, Bulgarian, Polish, Albanian, Bengali, Gujarati, Punjabi, Urdu.
