Clinical Assessor - Single Point of Access

We have an exciting opportunity for a Clinical Assessor to join our growing SinglePoint of Access team.
SinglePoint of Access is the central point for external referrals into Ashgate Hospice. As a Clinical Assessor, you will play an important role in assessing and prioritising referrals, identifying urgency and need, and ensuring people are directed to the most appropriate hospice service or support.
Working alongside the Lead Clinical Assessor, Care Coordinators and our wider clinical teams, you will use your clinical knowledge and professional judgement to respond to referrals, provide advice and guidance, and support effective care coordination.
This is also an opportunity to contribute to the continued development of a relatively new service. You will be encouraged to share ideas, contribute to quality improvement and help shape how our SinglePoint service works for patients, families and referrers
About the Role
You will assess referrals from a range of healthcare professionals and agencies, considering the person's individual needs, urgency and the most appropriate service response.
You will also provide advice and support to patients, families and healthcare professionals, working collaboratively with colleagues across Ashgate and with external partners to ensure people receive appropriate and timely support.
Key Responsibilities:
You will:
- Assess, prioritise and process referrals, responding appropriately to urgent and changing needs.
- Use your clinical knowledge and professional judgement to determine the most appropriate service or pathway.
- Provide advice, guidance and signposting to patients, families, carers and healthcare professionals.
- Work closely with Care Coordinators and patient\-facing teams to ensure referral outcomes are communicated and accurately recorded.
- Develop effective relationships with primary care, community, hospital and other healthcare professionals and organisations.
- Support the ongoing development of the SinglePoint assessment process, policies and pathways.
- Contribute to quality improvement, clinical audit, service evaluation and patient feedback.
- Maintain a high standard of clinical practice, documentation and professional communication.
- Work collaboratively with the wider multidisciplinary team to support responsive, person\-centred palliative and end of life care.
We are looking for an experienced and compassionate Registered Nurse or Allied Health Professional with experience of palliative or end of life care.
You will be confident using your clinical judgement to assess needs, prioritise referrals and make decisions in a timely way. You will be comfortable communicating with people who may be distressed or facing complex circumstances, and able to build effective relationships with patients, families and professionals.
You will be organised, adaptable and able to manage competing priorities while maintaining accurate records and a high standard of care.
Key Skills and Experience:
You will also have:
- Professional registration as a Registered Nurse or Allied Health Professional.
- Experience of working with people with palliative or end of life care needs.
- Experience of assessing and prioritising complex patient needs.
- Good knowledge of palliative and end of life care, including symptom management and Advance Care Planning.
- Excellent verbal and written communication skills.
- Experience of working collaboratively across multidisciplinary or multi\-agency teams.
- The ability to remain calm, professional and compassionate when dealing with distress, conflict or challenging situations.
- Strong decision\-making, problem\-solving and organisational skills.
- An understanding of safeguarding and the ability to recognise and escalate concerns appropriately.
- A commitment to continuing professional development, reflective practice and clinical supervision.
