Location: Rochdale (hybrid: office, home \& community)
Working Hours: 37 hours per week
Salary: £26,117 per annum
What You'll Be Doing

You'll work directly with unpaid carers, helping them access vital information and support to manage their caring role. This includes assessing needs, developing support plans, and connecting carers to services and peer support.
- Identify and engage carers, including those who may be 'hidden' within the community
- Carry out person\-centred Carers' Assessments in line with the Care Act 2014
- Provide tailored information, advice and signposting
- Support carers to access services, build networks and plan for emergencies
- Facilitate groups, drop\-ins, and community activities
- Maintain accurate records and contribute to performance targets
- Promote carers' rights and a whole\-family approach
- Deliver awareness sessions and attend community events

We need someone passionate about supporting unpaid carers who can provide professional and quality support.
You'll have:
- The ability to listen and communicate effectively with a wide range of individuals
- Experience of sensitively discussing and gathering information from people who want to access support
- The ability to provide individualised 1\-1 support to adults
