Join Our Team as a Domiciliary Care Coordinator
We are looking for an organised, compassionate, and proactive Domiciliary Care Coordinator to join our team and play a key role in delivering high\-quality care services to clients in their own homes.
As Care Coordinator, you will be responsible for ensuring our care packages are effectively planned, coordinated, and delivered safely. You will work closely with clients, families, care workers, and healthcare professionals to provide a reliable and person\-centred service that promotes independence, dignity, and wellbeing.
Key Responsibilities
- Coordinate and manage daily care schedules, ensuring all visits are appropriately allocated and covered
- Match care workers with clients based on needs, skills, and compatibility
- Monitor service delivery and respond promptly to changes or concerns
- Maintain accurate care records, rotas, and documentation
- Communicate effectively with clients, relatives, care staff, and external professionals
- Support recruitment, onboarding, and training processes for care workers
- Assist with care assessments, reviews, and quality monitoring
- Handle urgent situations and provide solutions to maintain continuity of care
- Ensure services meet company policies, regulatory standards, and best practice guidelines
- Take part in on call rota
We are seeking someone who is:
- Experienced in care coordination, scheduling, or a similar role within domiciliary care
- Highly organised with excellent attention to detail
- A confident communicator with strong problem\-solving skills
- Able to work under pressure and manage competing priorities
- Passionate about delivering excellent care and supporting vulnerable people
- Knowledgeable of care regulations and safeguarding principles
- Able to drive and have access to a car.
- Previous experience working within a home care setting
- Knowledge of care planning and rostering systems
- Experience supervising or supporting care workers
- Understanding of CQC requirements and quality standards

Pay: £25,000\.00\-£28,000\.00 per year
