Job Summary
We are seeking a dedicated and organised Care Coordinator to oversee and support senior care services within our care home. The ideal candidate will possess strong leadership and communication skills, with experience in supervising care staff and developing personalised care plans. This role involves coordinating daily operations, ensuring high standards of care, and fostering a compassionate environment for residents. A valid driving licence is essential, as the role may require travel between locations to support various care settings. The Care Coordinator will play a vital role in maintaining excellent service delivery and promoting the wellbeing of our residents.

Duties

  • Develop, implement, and review personalised care plans tailored to individual needs of residents

  • Supervise and support care staff, providing guidance and leadership to ensure quality standards are met

  • Coordinate daily schedules and activities within the care home to optimise resident satisfaction and safety

  • Liaise with families, healthcare professionals, and external agencies to ensure comprehensive care delivery

  • Monitor compliance with health and safety regulations, maintaining a safe environment for residents and staff

  • Organise training sessions for staff to enhance their skills and knowledge in senior care practices

  • Utilise IT systems to document resident information accurately and efficiently

  • Assist with emergency response procedures when necessary, ensuring prompt action is taken
Experience
  • Proven supervisory experience within a supported living or hospital environment

  • Qualified Nurse or have a clinical background

  • Strong leadership qualities with the ability to motivate and manage teams effectively

  • Excellent communication skills, both verbal and written, to liaise confidently with residents, families, and professionals

  • Experience in developing and managing care plans tailored to individual requirements

  • Competent IT skills for documentation and communication purposes

  • Valid driving licence is required for travel between different locations or appointments

  • Knowledge of health and safety regulations relevant to senior care settings

Pay: £40,000\.00\-£47,879\.78 per year
