As Care Coordinator, you will be at the heart of our service—ensuring care is delivered efficiently, safely, and to the highest standard. You will manage schedules, support care staff, and maintain strong relationships with clients and professionals.
Key Responsibilities
- Create and maintain person\-centred care plans tailored to individual needs
- Coordinate staff rotas to ensure consistent, reliable care delivery
- Match carers to clients based on needs, preferences, and continuity
- Liaise with families, clients, and healthcare professionals
- Monitor care quality and ensure compliance with standards
- Maintain accurate records and care documentation
- Support recruitment, onboarding, and training of care staff
- Respond to day\-to\-day operational challenges and provide solutions
- Ideally experience in a Care Coordinator or similar role within domiciliary care
- Or from a role or industry where you have to be organised and arrange rotas or planning as part of your daily role
- Strong organisational skills with the ability to manage multiple priorities
- Confident communicator with a professional and empathetic approach
- Ideally some experience working with some care system(s) and rostering
- Ability to learn systems and pick up new processes easily
- Capability to switch tasks and remain on top of workload whilst always focusing on being positive and calm
- Reliable, proactive, and solutions\-focused
