Budbrooke Medical Centre \- Care Coordinator

We are looking for a Care Coordinator to work for a friendly \& dedicated team of professionals at a GP practice on the outskirts of Warwick. We pride ourselves on providing the best possible service and care to patients and the community. We are looking for someone to be as equally committed and passionate in achieving this goal whilst developing their career and aspirations in healthcare. The ethos is one where we work as a team and help all patients with their needs.

What is a Care Coordinator?

Care Coordinators play an important part within the GP practice to proactively identify and work with patients, to provide coordination and navigation of care and support across health and care services.

They also help provide a service to the reception function, and work as part of the reception and administration team.

They support GPs day\-to\-day with patient care needs, and complete recalls for patients on routine screening or appointments.

Person Requirements:

*(ESSENTIAL)*

· A caring and compassionate individual who has an interest working in a health provision environment and with members of the public

· Flexible in approach and able to adapt quickly to new working practices

\- A 'can\-do' attitude and willingness to learn

\- Providing excellent patient service, with a compassionate, sympathetic understanding of all their needs

· Ability to plan and organise own work load

· Flexibility and ability to prioritise tasks

· Good work ethics

· Good team player

· Good communication skills (both verbal and written)

· Attention to detail

· Clear understanding of confidentiality within a health care setting

· Proficient in using IT systems and develop with the increased IT advancements in the NHS

· Flexibility to cover annual leave and to attend training courses where possible

\- Looking to develop a career in a healthcare setting

\- Ability to complete agenda's and meetings minutes

\- Highly organised and interested in change management

*(DESIRABLE)*

\- Previous experience within a healthcare setting

\- Previous experience with EMIS system

\- Customer service experience
