We are looking for a dedicated Care Coordinator to play a key role in ensuring our customers receive high\-quality, person\-centred support while helping to develop and support our care team.
As a Care Coordinator, you will be responsible for coordinating care services, managing staff schedules, conducting customer assessments, and maintaining compliance with care regulations and company standards. Key ResponsibilitiesScheduling \& Administration
- Coordinate weekly schedules for care support workers, ensuring effective coverage and continuity of care.
- Maintain accurate permanent hours and rota templates within QuikPlan.
- Monitor staff hours and performance against Key Performance Indicators (KPIs).
- Manage holiday requests and accurately record staff absences.
- Conduct initial assessments for new customers.
- Develop personalised support plans, risk assessments, and care documentation.
- Ensure care plans remain responsive to customers' changing needs.
- Act as a key point of contact for customers and their families.
- Communicate changes, updates, and cancellations effectively.
- Ensure all relevant information is updated within QuikPlan and PASS systems.
- Escalate concerns appropriately, including safeguarding issues, hospital admissions, and care\-related concerns.
- Maintain accurate customer information within PASS for care staff access.
- Conduct return\-to\-work interviews and support absence management processes.
- Organise and facilitate team meetings to encourage communication and engagement.
- Foster a positive, supportive, and professional workplace culture.
- Assist in delivering structured induction training for new Care Support Workers.
- Support ongoing staff development and learning opportunities.
- Participate in employee recognition initiatives, including "Above and Beyond" awards.
- Maintain accurate records and documentation in line with company policies and regulatory requirements.
- Support compliance with Infection Prevention and Control (IPC) procedures.
- Conduct audits and participate in supervision sessions and team meetings.
- Ensure high standards of care delivery and service quality.
- Support service growth through brokerage and referral opportunities.
- Assist with recruitment activities, including attracting and onboarding new Care Support Workers.
- Participate in the on\-call rota, providing support to staff and customers outside of normal working hours when required. (additional payment)
To be successful in this role, you will have:
Essential
- NVQ Level 3 in Health \& Social Care (or currently working towards it).
- Previous experience in a care coordination, senior care, or similar role.
- Strong organisational and time\-management skills.
- Excellent communication and interpersonal abilities.
- A compassionate, person\-centred approach to care.
- Strong problem\-solving and conflict\-resolution skills.
- High attention to detail and accuracy.
- Ability to work independently and collaboratively within a team.
- Full UK Driving Licence
- Experience using care management and scheduling systems, particularly QuikPlan and PASS.
- Good understanding of CQC standards, safeguarding procedures, and relevant care regulations.
- Experience conducting assessments and developing support plans.
